I’ve seen this term used a lot in really, really problematic contexts. So, my thoughts on the matter:

This is a person’s Personal Responsibility:

  • Things they do that hurt other people

This is true 

  • Even if it was an accident
  • Even if they had good intentions
  • Even if they don’t think it was a big deal

This is NOT a person’s Personal Responsibility:

  • Things other people do that hurt them

This is true

  • Even if someone says ‘but they wouldn’t have hurt you if you’d done X!’
  • Even if this is actually true for some reason 
  • Regardless of what X happens to be
